Family & Peer Support

Re-Entry & Community Reintegration
Support for individuals returning to the community, including resource navigation, employment support, housing navigation, and connection to community services.
Housing & Resource Navigation
Assistance identifying housing options, community resources, benefits, referrals, and other supports that promote stability.

Practical assistance with budgeting, household responsibilities, appointments, employment readiness, transportation planning, communication, and other skills needed for independent living.

Individualized assistance that helps people remain connected to resources, services, opportunities, and supportive relationships within their communities.
